HOA Resident Concern Notice & Repair Request

This convenient form may be used to alert us to a possible violation or bring our attention to an item that needs to be addressed by our maintenance personnel.

To report a possible violation of the Association’s CC&Rs, Bylaws or Rules and Regulations, please complete this Resident Concern Notice form and click on ‘submit’.

A Friendly Reminder Letter will be mailed to the appropriate party. Should the violation continue, you will need to submit a second notice. The Board of Directors will conduct an investigation of your report and, if warranted, a Violation Letter shall be sent to the offending party. If this action does not resolve the situation, you will need to submit a third notice. At that time the Board will issue a Hearing Notice to the offending party. Based on the findings of this hearing a fine may be assessed.

If the management company or the Board of Directors does not receive any additional complaints after the first or second notice, the matter will be considered resolved.
